Insurance & Billing

For patients having insurance with which we do not participate, we will gladly bill your insurance for covered charges. Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. If your insurance company has not paid your account in full within 45 days, we ask that you call your insurance company and check on your account. Should your insurance company only pay a part of our bill, the balance is your responsibility. We will gladly bill your insurance for you, but we cannot bill your insurance company unless you give us your insurance information. Please be aware that some and perhaps all of the services provided may be non-covered services. We will try to advise you if planned treatments fall into this category.

In insurance plans where we are a participating provider such as Medicare and Blue Shield, all copays are due prior to or immediately after the treatment. In the event that you change your insurance coverage to a plan where we are not participating providers, refer to the above paragraph.

Usual and Customary Fees

Our practice is committed to providing the best treatment for our patients and we feel our charges are fair. You are responsible for payment of your portion of any bill, regardless of any insurance company’s determination of “usual and customary” fees.
